Bring these to your first visit

  • Photo ID (driver's license or state ID)
  • Insurance card(s) — primary and secondary if applicable
  • Workers' comp or MVA claim number, if relevant
  • A list of current medications and dosages
  • Any imaging (X-ray, MRI, CT) on a CD or via portal — or the radiologist's name so we can request it
  • Referral paperwork from your primary care provider, if your plan requires one
  • Comfortable shoes and socks that are easy to remove

The new-patient packet covers history, medications, and consent. Filling it out at home cuts your check-in time to under 5 minutes.

Arrive 10 minutes early

If you didn't pre-fill the packet, plan to arrive ten minutes before your appointment so we can get you registered without delaying your provider time.

Confirm insurance the day before

Our front desk verifies coverage 24 hours ahead — if you switched plans, call ahead so we can re-run benefits.
