What is total ankle replacement?
Total ankle replacement (total ankle arthroplasty) is a surgical option for patients with severe ankle arthritis who want to preserve ankle motion rather than undergo a fusion. Modern implants have dramatically improved over the past two decades, and for the right candidate they offer comparable pain relief to fusion with substantially better long-term function. Performed at NES by our surgical podiatry team — Dr. Mia Horvath, Dr. Thomas Melillo, and other board-certified foot and ankle surgeons depending on case complexity.

Common reasons to see total ankle replacement
How we diagnose
Total ankle replacement evaluation starts with a thorough exam — alignment, motion, ligament integrity, and a careful look at the joint above and below the ankle. Weight-bearing X-rays are taken in our office, often supplemented by CT or MRI to assess bone quality and surrounding structures. We discuss whether you are better served by replacement or fusion based on your age, activity level, alignment, and the condition of the surrounding joints. Most patients leave the consultation with a clear understanding of both paths and the trade-offs of each.
Treatments we offer
Total ankle replacement is not for every patient with ankle arthritis. Patient selection is everything — and we will tell you straight whether replacement, fusion, or continued conservative care is the right call for your specific situation.
